Some of the most common symptoms of Polycystic Ovarian Syndrome include irregular periods, weight gain, acne, unwanted hair growth, infertility, mood swings, and hair thinning or loss.

*This survey is a Polycystic Ovarian Syndrome (PCOS) Screening Tool to aid pre-menopausal women and non-PCOS specialized providers in determining if patients should seek PCOS Diagnosis with a PCOS Specialist. This survey is not a PCOS diagnostic tool. PCOS diagnosis requires blood work and a medical visit. This tool is also appropriate for use by non PCOS specialized providers ie primary care and mental health.
